Pet Travel Question: Exporting a Cat from the US

My kitten is about three months old, and we are traveling to Colombia. She recently got her rabies vaccine, and my current vet is not USDA certified to sign/complete a health certificate as is requested by JetBlue (the airline I will be traveling with). If I go to a certified vet and complete this health certificate, do I have to send this certificate somewhere or just show it to the airline?

As you'll see, in addition to being completed by a USDA-accredited vet, the vet health certificate must also be endorsed by the USDA.
