Pet Travel Question: Flying a Dog to Germany and Back

We are flying a private plane to Germany in May for a few weeks and returning back to the USA. We would like to take our dog. I know that I need an EU Passport & Microchip, but is there anything else that I need for her? What do they require for returning to the USA?
