Pet Travel Question: Moving Older Dogs

I may move to Antigua within the next six months. I have a 14.5 year old OES/Border collie mix dog. She is very healthy but is very serious and loves her routine. What is the best way to move my dog to Antigua that will be the least amount of stress for her?
