Snub-Nosed Breed Release

Customer represents that Customer is aware that when snub-nosed breeds are transported and/or lodged they may experience a great deal of stress due to the hereditary weaknesses of their respiratory system and the extreme risk involved when flying a snub-nosed breed. This stress may cause physical complications for the pet which may result in illness and/or death of the pet.

Waiver of Liability and Hold Harmless

Customer agrees that PetRelocation LLC shall not be held liable for any loss, injury, including but not limited to death, or any other damage sustained by pet during the relocation process. It is Customer’s express intent that this form shall be deemed as a release, waiver, discharge and covenant not to sue PetRelocation LLC, its trustees, officers, agents, and employees.

Customer acknowledges and represents having read this Notice, including the Waiver of Liability and Hold Harmless provisions contained herein, and Customer fully understands this Release Form and signs it voluntarily. Customer further acknowledges that PetRelocation LLC has made no oral representations, statements, or inducements.
